my name start miss kahn in this quick video on the cover you form creation
fairly simple to do and there's two ways you can do it at the election you form
creator rate within oskar if you go to the administration section
scroll down and body forms click uploader form

want actually find the image to put it there on the Select the orientation

again with my photo editor at nature that its 1500 pixels wide
and I and click Load image and lo and behold there it is

click the mail check box amor and click on the top left over you want to check

today so there's three different types of inputs their single line text inputs
multiline text input
and checkboxes the first one right up here this is where the patient's name is
supposed to go
so we are gonna do a single line text input
and we're gonna auto populate that text box
from the Oscar database where the person's last name common first name
and then I draw out my text box and i'd
start at the top left over I want to go and drag it all the way down to the
bottom right
and there that's a new Tom
text box that's gonna be generated by the form generator
that I is can overlay over top of this image
an HTML and is going to pull
an the data that I selected from here so the last name first name from the
patient's chart
so we'll continue on the next thing that we want is the date of birth
so scroll down to the data for his patient
are gonna happen day month year format and again we're in
top left to bottom right over you want this to go
good next thing we want is some checkboxes
sell on that bill to I'm gonna click again on the top left of where you want
these checkboxes to go
looking good and then we want %uh personal health number of this person's
again that a single line text input

you can name the input fields however you'd like but
you can just use the standard settings and all worked just fine for him

and that pops up a new Firefox window
om you can click Firefox link up top and you can't save the page as
and a favor to the same folder and we're gonna save it as

double click on it and then click Upload I've already uploaded this image
com it is right there but to just make sure that your image get uploaded
as well as he reform click back
answer now when I go ahead and click

for my text boxes and checkboxes and
you can see that nothing's been pre filled up here if I go ahead
and exit out of this and then go to a patient's chart

paper form they have to fill out any camp refill it with all your patients
