If you feel you were assessed incorrectly on any items appearing on your Remittance Advice or would like to make updates to a claim previously submitted, you are able to submit inquiries on items that are not coded FD (Final Decision) and are not older than 4 months from the date of the RA of the ministry's payment decision letter.
To dispute a final decision a request for a hearing by the Physician Payment Review Board must be made within 20 days.
All RAI must be filled out in the appropriate RAI form and mailed or faxed to OHIP for review.
You will need to fax the form to the new centralized fax system at 905-434-4186.
