This eForm is for use within the Vancouver Coastal Health Authority. It includes both the Magnetic Resonance Imaging (MRI) Patient Screening Form and the MRI Contrast Agent Questionnaire. These questionnaires are used with patient before MRI scans, and contain questions related to patient's current health situation, and serve as consent form prior to the examination being taken.
