Billing Forms in OSCAR allow you to create lists of service and diagnostic codes that can be easily accessible when creating your invoices. You can create multiple billing forms and switch between them on the invoice creation page. Providers can also select their main default billing form in their preferences so that it shows when invoices are created for their patients.


Note that billing forms are a system wide setting. If one of your user modifies a billing form, it will change is for all users of your system.


Editing an Existing Billing Form

1. From the schedule page, click on "Administration" to open the admin panel.

2. On the left, click on "Billing" to expand that part of the menu, then click on "Manage Billing Form".

3. Using the radio buttons, select if you want to edit a service code or a diagnostic (Dx) code billing form. Select the form you would like to edit using the drop down menu, then click the "Manage" button.

Note: Each form require a service code form. Diagnostic (Dx) code forms can be attached to existing service code forms. The service code will display codes available to select directly on the invoice page while the diagnostic code will be a simple list of common diagnostics.

4. a. Editing the service code form: 

You will find three columns on the edit page, each with their own header. Each of the column represents a section of the invoice creation page. You can edit the header of each column by simply changing the wording in the top fields (identified in orange below). The service codes themselves are located on the left side of each main column, as identified in blue below. The right side, identified in green, is used to determine the order each of these codes will be displayed in on your invoice creation page. In my example below, the code 12120 identified as position 12 under the 'Visit' column will be showing above code 00103.

b. Editing the diagnostic (Dx) code form:

The diagnostic code form looks slightly different. It is still laid out in three columns in which you will add your diagnostic codes. The order the codes are showing on your screen depends on the order you enter them in the list. The full content of the first column will be displayed before the first code on the second column, and so on.

5. Once you are done editing the form, click the "Update" button located at the bottom left of the page.



Creating a New Billing Form

1. From the schedule page, click on "Administration" to open the admin panel.

2. On the left, click on "Billing" to expand that part of the menu, then click on "Manage Billing Form".


3. Using the radio buttons, select if you want to create a service code or a diagnostic (Dx) code billing form. In the drop down menu, select the option "Add/Edit/Delete Form", then click the "Manage" button.

4. On the new page, fill in the "Adding New Form" section.

Service Type ID: This is an internal ID. It can be up to three letters long.

Service Type Name: This will be the name that will identify the billing form in the invoice creation page.

Group Names: These are the headers of each column. If you are unsure of what you would like to use as headers, these can be modified later by editing the form.

5. Click the "Add Form" button once you are done.



Deleting a Billing Form

1. From the schedule page, click on "Administration" to open the admin panel.

2. On the left, click on "Billing" to expand that part of the menu, then click on "Manage Billing Form".


3. Using the radio buttons, select if you want to create a service code or a diagnostic (Dx) code billing form. In the drop down menu, select the option "Add/Edit/Delete Form", then click the "Manage" button.

4. On the new page click on the 'Service Type' associated with the form you would like to delete in the list on the right. A pop up will ask you to confirm you wish to proceed. Click 'Ok' and the form will be deleted.
