To support HPV related improvements rolled out by Ontario Health and OntarioMD, several updates have been made to Juno EMR. The sections below outline these changes; how to access the new eForms, when they become available, and some tips for quicker eForm access.
New HPV Prevention Entry
Starting March 3rd, 2025, a new prevention item is available on all systems called "HPV Test."
The prevention follows the standard "Screening" layout available in Juno EMR.
New HPV eForms Available for Use
Two new eForms have been made available. You can find those eForms at the bottom of this page, and follow the instructions below to add those eForms to your Juno EMR system.
Please note that OntarioMD and Ontario Health have requested that these not be made widely available before March 3rd, 2025, as Lab Services may not yet have the capacity to handle the requisitions.
eForms can be added to Juno EMR either as zip files (with all the components of the eForm) or as separate file components (image files and HTML/ Javascript code). There are two separate ways to add the different file types to your account, and will be done in the admin panel.
In the Classic UI, the Admin Panel can be accessed by clicking on the "Administration" link at the top of the schedule page.
To access the Admin panel in Juno UI, click on “Admin” at the top of the schedule page. The Admin tab may be hidden under the “More” option if you are using Juno with a smaller screen display, click on “More”, then click on “Admin” from the dropdown.
Note that eForms can only be added by users with admin access.
Adding a zip file
In the Admin panel, you can either:
click on the "Manage eForms" tile to the right, or
click on the "Forms/eForms" tab on the left then click on the "Manage eForms" link from the dropdown.
Click on the "Import" button on the left of the eForm Library page (second button from the left).
Click on the "Browse" button, to find and select the zip folder from where it's saved.
After selecting the zip folder, click the "Import" button to the right of the file name.
Once the zip folder is imported, the eForm should be listed in the eForm library (on the same page).
Accessing the eForms on a Patient Chart
- Click Add+ in line with the Forms section of the patient's Summary page or click the Forms tab in the patient's eChart and select Form Library.
- Search for the form using the search bar at the top of the Patient Forms list.
- Click the name of the eForm that you would like to use.
eForm Groups
You may also want to consider adding the eForm to a group for ease of access. To add the eForm to a group:
- Click Admin.
- From the left-hand menu in the Administration Panel, click Forms/eForms.
- From the menu that appears below, click eForm Groups.
- Select the name of the group to which you would like to add the eForm (or add a new name if you are creating a new group).
- Click Add eForm.
- Locate the eForm you would like to add to the group.
- Click Add eForm to Group.

You can select a Group from the left-hand side of the Forms tab of the eChart to filter the available eForms to just those listed in that group.
